Appointments

Appointments can be made by calling our office during normal business hours 8 am to 5 pm.  We are at this time accepting new patients

After Hours & Emergency Care

If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day after 5:00 pm,  7-days a week.  Please call our office at 940-723-BABY (2229) 

Prescription Renewals

Prescriptions are renewed during normal office hours. In some instances, we may require that the patient be seen in our office prior to medication renewal.

Financial

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards, and paypal to email address cghanbari2@gmail.com .  Please contact our billing office should you have any financial questions or concerns.

Billing & Insurance

We participate with most local and most national insurance plans, HMO and Tricare. We do accept Medicaid:  Firstcare, Amerigroup, Superior, Traditional Medicaid, Superior Chips, Molina and Texas Healthy Women. We will accept patients that have applied for Medicaid and are waiting to be approved.   

We do accept self pay patient.  Please contact the office for prices.

Please make sure to bring your insurance card with you and also another form of identification.  We will also need the social security number of the person the policy is under.   Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.
